Overview

Atlantic Health System is seeking a Inpatient Care Manager for their Overlook Medical Center location. This incredible opportunity is a Per diem, day shift.



Responsibilities
  • Essential Functions/Responsibilities of Position:
    • PD RN Case Manager; review for level of care and acuity using approved evidenced based guidelines; discharge planning
    • Establishing initial plan of care and discharge plan; completing initial and ongoing assessments
    • Re-evaluating and completing discharge planning with CMS COP guidelines
    • Ongoing communication with patient, family, healthcare team
    • Managed care provider while adhering to CMS guidelines


Qualifications

Qualifications:

Minimum Education:

  • High School Diploma or GED required
  • BSN required

License/Certifications:

  • Current licensure in the state of NJ
  • Certification in Case Management specialty preferred

Relevant Experience:

  • 2 years Hospital case management
  • 2 years managed care experience
  • Knowledge of Medicare and managed care payors, EPIC (EMR) experience preferred
  • 2 years acute care inpatient hospital experience
