The Home Based Primary Care (HBPC)/Patient Aligned Care Team Clinical Pharmacy Practitioner is a licensed pharmacist, a Pharm.D. Degree and/or a completed residency are preferred. The HBPC /PACT Practitioner is responsible for providing clinical pharmacy services and managing all aspects of the Home Based Primary Care Program at the NY Harbor Healthcare Systems New York Facility. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education:(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/.(NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). c. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment in paragraph 2, the following criteria must be met when determining the grade of candidates. GS-13(a) Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, Pharmaco-economics or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: PGY1, PGY2 Geriatrics, Geriatric Board Certification References: VA Handbook 5005/55 Part II Appendix G15.Licensed Pharmacist Qualification Standard The full performance level of this vacancy is GS-13. Physical Requirements: See VA Directive and Handbook 5019. ["Including but not limited to policy development, quality resources and coordination of educating staff to optimize cost effectiveness and patient outcomes. This may include participation on various committees pertaining to HBPC, reviewing charts and drug therapy for patients, performing clinical assessments and interpreting lab data, providing drug information services including those services requiring extensive interpretation, providing pharmacokinetic consults as clinically privileged, attending home visits to educate the patient and family on pharmaceutical care, and providing inservice educational presentations to medical center staff. Extensive documentation and clinical progress note writing are required of this position. A. Clinical: 1. Responsible for developing, organizing, coordinating and managing all aspects of clinical pharmacy functions for HBPC, attends and participates in HBPC multidisciplinary rounds, team meetings, conferences, and other meetings as required for the purpose of making recommendations and providing consultation in all Pharmacy-related matters. 2. Responsible for all HBPC patients registered in the program, must work independently and effectively to assure appropriate, accurate drug reviews are completed; interpret prescriptions and/or physician orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; reviews patient medication profiles, contacts physicians and makes authorized changes in orders or prescriptions as required. 3. Functions independently in day-to-day clinical and administrative duties associate with the position, consulting with higher level supervisors only when required. In general, uses a high degree of appropriate professional judgment to solve complex problems, which routinely arise throughout the work day. 4. Demonstrates recognized expertise in the area of geriatrics and the HBPC Pharmacy program. The HBPC /PACT Practitioner interacts with physicians and other health care professionals as demonstrated by regular attendance at HBPC multidisciplinary team meetings, medication review rounds, and other meetings for the purpose of providing consultation in all Pharmacy related matters. 5. Works closely with physicians and nurses to monitor patient response to medications, including efficacy, safety, and outcome, with special attention to polypharmacy problems in the elderly. Submits summary data or documents a significant impact of interventions on pharmaceutical care, costs, patient compliance, reduction in morbidity/mortality, selection of formulary products, reduction in numbers of medications, etc. B. Educational Responsibilities: 1. Demonstrates knowledge of the standards related to Home Based Primary Care, Ambulatory Care, and Nursing Home drug use. This includes, but is not limited to, the standards established by DEA, FDA, VA, the state, and JCAHO. 2. Serves as an authority on drug usage, interactions, over dosages, and compliance to medical staff, nursing staff, technicians, aides, students, and pharmacists of lower pay grade. 3. Responsible for keeping abreast of changes in the Pharmacy Service (I.e. Formulary additions and deletions, interdepartmental memoranda, relevant station memoranda, and policies and procedures) and to maintain a general knowledge of VA, DEA and JCAHO regulations. 4.Contributes to Pharmacy Newsletters and other educational materials as assigned. 5. Participates in P&T Committee functions and assignments as required, e.g., ADR monitoring, medication error reporting, Medication Utilization Evaluation activities, drug monographs/reviews, etc. 4. SUPERVISORY CONTROLS The incumbent is administratively accountable to the Associate Chief, Clinical Pharmacy Service, or their designee, for professional licensure, competency and continuing education. The HBPC /PACT Practitioner will function independently in regard to the administrative aspects of the various functions of the position. The incumbent will be expected to act as supervisor and coordinator to students and residents assigned, functioning as the pharmacy expert for policy, therapeutic or complex problems.
