This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. BASIC REQUIREMENTS:United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Handbook 5005, Part II, Chapter 3, Section A, paragraph 3.g. Education: (a) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/.(NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (b) Foreign degrees: Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). GRADE DETERMINATION: Clinical Pharmacy Specialist, GS-13 (a) Experience. In addition to the basic requirements, must have 1 year of experience equivalent to the next lower grade level. (b) Demonstrated Knowledge, Skills, and Abilities (KSA). Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Below are examples of open positions at the Houston VA Medical Center: Within a scope or practice, provides expert pharmacotherapy services for patients and professional staff in the specialty area and as assigned to optimize patient response and resource utilization: Primary Care Clinical Pharmacy Specialist Cardiology Clinical Pharmacy Specialist Emergency Department Clinical Pharmacy Specialist Hematology/Oncology Clinical Pharmacy Specialist Internal Medicine Clinical Pharmacy Specialist Infectious Disease Clinical Pharmacy Specialist Surgery-Inpatient Clinical Pharmacy Specialist Mental Health Clinical Pharmacy Specialist Pain Management Clinical Pharmacy Specialist Spinal Cord Injury Clinical Pharmacy Specialist Substance Use Disorder Clinical Pharmacy Specialist Left Ventricular Assist/Heart Transplant Clinical Pharmacy Specialist In addition to the specific duties and responsibilities, all pharmacists' duties include, but are not limited to: Functions as a provider in the provision of evidence-based pharmaceutical care services through direct patient care within a scope of practice provided by privileges accorded by credentialing and privileging Attend and actively participate in patient care rounds and/or huddles as assigned Evaluates providers' orders as to appropriateness based on patient medication profiles, inputs information into VistA or verifies orders entered by healthcare providers as needed Assures the safe, appropriate, cost-effective use of drugs in assigned patient care areas through the application of specialized knowledge and the use of judgment in the collection and interpretation of data specific to each patient and in tactful interactions with other professionals Serves as an effective liaison between Pharmacy Service and other professional disciplines and services for consultation regarding drugs, pharmaceutical problems and pharmacy operating policies and procedures through barrier resolution and active problem-solving skills. Recommends change of procedures when necessary to accomplish medical center mission and goals. Helps develop and work within the quality assurance guidelines for the pharmacy service and Michael E. DeBakey VA Medical Center as assigned Works with pharmacy administration to assist in development of action plans to address drug shortages, recalls, lost opportunity cost initiatives; etc. Evaluates prior authorization drug requests through in-depth evaluation of the patient's condition and recommends alternatives when indicated and completes consults within established timeframes Develops and provides education programs for patients, pharmacy, medical, and nursing staff Monitors and reports drug errors, adverse drug reactions, and allergies, per MEDVAMC policy and procedures. Assists pharmacy adminstration in planning and implementing clinical training through lectures, discussions, seminars, patient-rounds, in-services, and other didactic and practical experiences as requested Participates in medication use evaluations for local, VISN, and National and other quality assurance programs as assigned by pharmacy administration Serves on national, VISN and local committees as assigned by pharmacy administration Works with providers to ensure compliance with national, VISN, and local initiatives Prepares and presents drug information to support the pharmacy service and Pharmacy and Therapeutics Committee as assigned by pharmacy administration Supplies coverage to assigned pharmacy areas as needed Work Schedule: Shifts may vary based on the needs of the service with occasional weekends and holidays; Work schedule will be discussed during the interview process.
