LOCATION IS IN EVERETT, WA. The Home Based Primary Care (HBPC) Clinical Pharmacy Specialist (CPS) functions at the highest level of pharmacy clinical practice, providing evidence-based comprehensive medication and disease state management and pharmaceutical care services to Veterans. To qualify for this position, applicants must meet all requirements within 30 days of the closing date of this announcement. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. (Home Based Primary Care) Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. (Home Based Primary Care) Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters Preferred Experience: PGY2 in ambulatory care or equivalent (PGY1 + 3 years), working under scope of practice preferred, experience in geriatrics highly recommended. The actual grade at which an applicant may be selected for this vacancy is GS-13. Physical Requirements: Heavy lifting, 45 pounds and over; Moderate lifting, 15-44 pounds; Light lifting, under 15 pounds; Moderate carrying, 15-44 pounds; Reaching above shoulder; Use of fingers; Both hands required; Standing, 6 hours; Repeated bending, 2 hours; Near vision correctable at 13" to 16" to Jaeger 1 to 4; Far vision correctable in one eye to 20/20 and to 20/40 in the other; Depth perception; Ability to distinguish basic colors; Ability to distinguish shades of colors; Hearing (aid may be permitted); Mental & Emotional Stability; Working near/around constant noise and dust; Extensive Keyboard/mouse use; Working closely with others; Working alone; Protracted or irregular hours of work; ["VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEwsClinical: Works autonomously and collaboratively with the HBPC Interdisciplinary Team (IDT) to provide comprehensive medication and disease state management services and prescribing medications and treatments using approved scope of practice at least 25% of the time to manage medication therapy and disease state outcomes. Specifically, the CPS will: Obtain pertinent medical information to include health and medication history. Perform basic physical assessments, during home visits when appropriate, to obtain objective information needed to assess the safety and efficacy of the Veteran's medications along with their disease state. Order laboratory testing based on clinical assessment and recommended medication therapy safety and efficacy monitoring parameters. Design, implement, modify, and document patient specific pharmaceutical care plans based on information obtained by the aforementioned. Initiate, modify, and discontinue medications as appropriate to achieve patient specific therapeutic outcomes. Communicate the pharmaceutical care plan to the Veteran, their family and/or caregiver, and the HBPC IDT. Communicate significant findings with other health care providers to ensure continuity of care. Preforms a medication assessment of medication therapy initially, quarterly, and when clinically indicated, to include recommendations that ensure a custom approach that supports patient preference and adherence. Participates in home and telehealth visits, when clinically indicated and agreed upon, for comprehensive medication or disease state management or medication safety concerns. Documents HBPC medication reviews, assessments, recommendations, and interventions in the electronic medical record. Provides dosing recommendations including pharmacokinetic dosing, based on age, renal or hepatic dosing; usual recommended starting doses, titrations, and maximum dosing to optimize disease state management, conversions to formulary agents, and anticoagulation management. Communicates, adjudicates, and completes formulary changes, drug safety alerts, drug shortages, and drug conversion plans made by the VHA Pharmacy Benefits Management; Pharmacy Nutrition and Therapeutic Committee, Pharmacy Clinical Service Line and the VAPS Formulary Manager. Educational: Works directly with patients, families, caregivers and HBPC IDT to provide education necessary to achieve the desired outcomes. Conducts comprehensive medication reviews, counseling and education for Veterans, their family members, and HBPC staff. Provides drug information and reference materials to health professionals and pharmacy staff regarding changes in practice, formulary management, and new therapy modalities within their area of practice. Acts as a clinical preceptor and mentor for VA Puget Sound PGY1 and PGY2 pharmacy residents and Doctor of Pharmacy students. Coaches and mentors other clinical pharmacists. Administrative: Collaborates with pharmacy supervisors, leadership, and staff to develop processes for more efficient prescription processing and formulary management within pharmacy services, specialty areas of practice and HBPC. Participates in various continuous quality improvement and assurance (CQI) initiatives including medication use evaluation (MUE) and the collection and analysis of clinical outcomes data. Research: Assists medical staff in developing, conducting, and presenting the results of medication use evaluations and outcome measures. Participates as a preceptor for pharmacy resident projects. Provides investigational drug information to medical, nursing and other professional staff and investigational drug counseling to Veteran patients when appropriate.
