General Description of the Job Class

The Continuity of Care pharmacy technician is a self-motivated, independent advanced pharmacy technician responsible for working collaboratively with the Transitions of Care Manager within the Clinical and Patient Care Services division to collect medication histories for patients admitted at Duke University Hospital. 

Duties and Responsibilities of this Level

• Use high-level communication skills to perform patient interviews and obtain a complete and accurate home medication list. This includes contacting retail pharmacies or long-term care facilities to verify the medication history with persistence to achieve the highest level of accuracy.
• Ensure complete and accurate documentation for each identified medication in the electronic medical record with careful attention to detail.
• Discuss any identified issues related to the medication history with the clinical pharmacist staff to promote high-quality transitions of care.
• Provide medication history related pharmacy technician and student training and development.
• Participate in departmental goal planning and quality improvement activities, focused on care transitions
• Maintain familiarity with all departmental programs
• Perform other related duties incidental to work described herein

• Knowledge of medication names (brand/generic) and basic math.
• Attention to detail and persistence in work ethic
• Able to work independently and prioritize responsibilities in a high-volume environment
• Necessary communication and interpersonal skills to maintain effective working relationships with patients and co-workers
• Takes responsibility for personal and professional growth
• Must be able to operate a computer, telephone, pager and other pharmacy equipment

Minimum Qualifications

Education

• High school diploma or equivalent required
• Completion of a certified pharmacy technician training program preferred

Experience

• Minimum two years pharmacy technician experience, but 5 years (Community/Retail setting preferred)
• Experience performing medication histories/reconciliation is preferred

Degrees, Licensures, Certifications

• Pharmacy Technician Certification Board (PTCB) certification required, unless actively enrolled in an accredited Doctor of Pharmacy program

Must be eligible for registry with the NC Board of Pharma
