Position Description:

The social worker/therapist is part of a multidisciplinary team that includes medical providers, nurses, and case managers. They will be collaborating with service providers from Homeless Health Outreach and Mobile Engagement (HHOME) team and the Getting to Zero (GTZ) intensive case management programs. This position will function as a clinical leader on the team, working to engage clients with acute mental health and medical challenges into mobile care. This position is responsible for providing behavioral health services to the agency’s clients who are trying to manage their mental health and substance use. The social worker/therapist will carry a caseload of therapy patients and will provide individualized social work services to clients so that they can achieve more their personal and wellness goals.

  • Conduct field-based intake, assessment, and reassessment of clients with mental health and substance use disorders
  • Provide consultation to the HHOME and GTZ programs for urgent situations assist with treatment planning and development
  • Be a liaison between all psychosocial services and clients’ medical provider to ensure that clients’ care is coordinated
  • Develop and utilize multiple modalities to address clients’ substance use and mental health needs, including but not limited to individual psychotherapy using evidence-based treatments
  • Continually update client care plans and develop appropriate discharge while coordinating with outside parties including hospital social work teams, psychiatric emergency and urgent care etc.
  • Provide Clinical Supervision to GTZ and HHOME team staff as needed
  • Participate in all required meetings, including daily morning huddles and case conferences
  • Provide service referrals, linkages, care coordination, and advocacy services for clients, and monitor the outcomes of these activities
  • Help care team tailor health education for the client and provide support for medication adherence
  • Provide referrals to medical and psychosocial service resources and programs when needed
  • Complete necessary documentation, assessments, and treatment plans in a timely manner
  • Lead and participate in case conferences, monthly staff meetings and in-service trainings
  • Participate in and comply with agency quality assurance, peer review, clinical consultation
  • Work collaboratively with an interdisciplinary team of medical services, medical case management, prevention and testing services, and other internal and external social support programs
  • Perform other duties and tasks as assigned

Minimum Qualifications:

  • Master’s Degree or equivalent required
  • ASW registration required or equivalent required. License preferred
  • Clinical expertise in assessing and managing the psychosocial needs of low-income populations, multiply diagnosed patients (HIV, mental disorder, and substance abuse), LGBTQI folks, immigrants and refugees
  • Experience with harm reduction, client-centered counseling, motivational interviewing, and other evidence-based treatment approaches
  • Experience working in interdisciplinary teams. Must be able to work independently and have an adaptable working style and be a strong contributor to team performance
  • Demonstrated reliability and good interpersonal, organizational, verbal, written communication skills. Must be able to handle multiple tasks and have ability to work professionally and ethically within multi-racial and multicultural settings
  • Be able to provide customer service in an effective, courteous, and friendly manner while meeting the needs of diverse clientele
  • Knowledge of legal mandates and ethical issues related to client privacy and confidentiality
  • Ability to travel within San Francisco and, at times, the Bay Area or nationally
  • When eligible, completion of an Involuntary Hold/5150 certification
  • Fully vaccinated against COVID-19 and able to show proof of vaccination + Booster if qualified
  • Must be able to sit, stand, and walk for extended periods of time
