In fiscal year 2018, Duke University Hospital admitted 42,916 patients and had 1,085,740 outpatient visits in fiscal year 2017.

Duke University Hospital is ranked in the top 20 nationally for seven adult specialties, including cardiology and heart surgery, nephrology, ophthalmology, orthopedics, pulmonology, rheumatology, and urology.

Duke University Health System Job DescriptionCLINICAL PHARMACIST, PER IOPERATIVE SERVICES (5079)Job Title: Clinical PharmacistJob Code: 5079Ge neral Description of the Job ClassThe perioperative clinical pharmacist is responsible for oversight ofthe medication managementprocess in perio perative services at Duke University Hospital(locations include the Oper atingRoom (OR) Pharmacy Satellites operations in DukeNorth,Duke Medici nePavilion, the Duke EyeCenter, and all associated operating rooms and t he post-anesthesia careunits (PACU).The perioperative clinical pharmacis t will be responsible for applyingestablished medicationmanagement polic ies including drug distribution, inventory control,waste reduction, drug useoptimization, formulary enforcement, clinical services, qualityassur ance, and involvement ineducation and training of learners. The clinica l pharmacist will serveas a resource to the surgery,nursing, anesthesia, and ancillary team members. They will strive tomaintain the higheststa ndards in customer service and foster positive and collaborativerelation ships with theperioperative team members.The clinical pharmacist must be able to display effective communicationtechniques, use strategicand cri tical thinking skills, and utilize effective clinical andoperations skil ls to triage problemsappropriately. The pharmacist must have knowledge a nd understanding of:automated dispensingcabinets and their functionality , surgical care improvement project(SCIP), electronic healthrecords and the use of computerized physician order entry (CPOE), painand controlled substancemanagement in the inpatient critical care setting, USP 797, ph armacyinventory management, andan understanding of medication safety con cepts.Duties and Responsibilities of this LevelThe following activities are components of the position to assist inmeeting the major jobresponsi bilities:Review and monitor patient's medication therapy and orders writ ten byappropriate caregivers forsafety and efficacy considering specific patient conditions and healthstatus; advise patient careteam of issues with, or possible enhancements to, patient's medicationtherapy and docum entsuch interventions. Prepare, label and dispense medications forpatien ts in accordance withphysicians' prescriptions; ensure compliance with F ederal, State andlocal laws and regulationspertaining to the dispensing of drugs and narcotics and the maintenanceof required records.Responsibl e for being familiar and stay abreast of information relatedto all drugs used in thesetting, including drugs for general and regional anesthesia ,neuromuscular blockade, analgesia,preoperative management, hemodynamic control, diagnosis andmanipulation during surgery,prevention of infectio n,treatment of intraoperativeemergencies, controlof bleeding, andpostope rative nausea and vomiting (PONV). Ensure compliance with controlled sub stance distribution and controlsystem (including method ofdistribution, records maintenance, ordering, storage, access,reconciliation, disposal, reporting andmanagement of discrepancies/variances) Provide drug inform ation and educate caregivers, patients and studentsregarding medications andtheir proper use and monitoring. Oversee and monitor all aspects of themedication process;collect data and presentreports and recommendatio ns regarding issueswith the medicationsystems.Ensure that appropriate me dications are available for patients andproperly procured, prepared,stor ed and maintained.Supervise the process and technical personnel who assi st inpreparation, maintenance anddispensing of drugs and supplies. Prov ide guidance to technicalpersonnel as needed to maintainbest practices.P articipates in orientation, training and continuing education programsfo r clinical pharmacists andpharmacy technicians.Demonstrate collaboration with inter and intra-departmental workgroups.Serve as a mentor and prec eptor for prospective students and residents.Maintain currentknowledge a nd critical thinking skills for managing drug shortages inthe inpatientp erioperative setting. Minimum Qualifications

 

 

Education

Doctor of Pharmacy Degree OR BS Pharmacy Degree

 

Experience

 

With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required. With Doctor of Pharmacy degree, no experience is required.

 

Degrees, Licensures, Certifications

 

Licensed Pharmacist or eligible to practice pharmacy in North Carolina
