Job Summary

This paragraph summarizes the general nature, level and purpose of the job.

The Resource Center Manager (RCM) oversees the practice of the Resource Center Care Coordinators and is responsible for providing patients and families with the guidance, resources and information needed to navigate a complex health care system. S/he is responsible for identifying, delegating and overseeing functions related to discharge planning, utilization management and patient care coordination by communicating and collaborating with the Case Managers, Social Workers and other interdisciplinary team members in coordination of resources and services. The RCM will communicate with the Director of Social Work, Internal Department Managers, review organizations, vendors, providers and other external customers in an effort to provide education, and to enhance the resources and support services available to families. The Resource Center Manager is responsible for establishing and maintaining systems that ensure all traveling families have a variety of low-cost housing options and transportation to and from the hospital for low-income families who have no reliable means of transportation. The Resource Center Manager initiates and fosters private, corporate and civic relationships that will result in increasing the Department's no-cost and low-cost housing and transportation options, including the Care-A-Van Program.

Essential Functions

Employees must abide by all Joint Commission Requirements including but not limited to sensitivity to cultural diversity, patient care, patients rights and ethical treatment, safety and security of physical environments, emergency management, teamwork, respect for others, participation in ongoing education and training, communication and adherence to safety and quality programs, sustaining compliance with National Patient Safety Goals, and licensure and health screenings.

Must perform all duties and responsibilities in accordance with the Service Standards of the Hospital(s).

  • Collaborates with families, physicians, nurse practitioners, directors and managers, schedulers and other key people involved in the daily activities of providing medical care to patients at LPCH, gathering information to assess any barriers to care as well as resources available to assist families.

  • Collaborates with the Director of Care Management, creation of databases, oversight of data entry process and integrity of databases as required to assess and improve processes. Prepares reports for distribution as appropriate.

  • Identify, track and trend metrics for the care coordination team

  • Works with Care Manager, Media Relations and Resource Center staff in developing tools to assist families in obtaining the resources they need.

  • Participates in the development and implementation of new programs, operating protocols and/or communication initiatives related to coordinated care.

  • Serves as the liaison between Resource Center staff, families, other departments and the organization regarding needs for education, clarification, referrals and resources.

  • Assists staff with compiling resource information and developing files to standardize information and resources available to families.

  • Speaks with families to listen to suggestions and to assess financial and resource needs, advise and counsel as needed, educating them to available resources and support.

  • Communicates information on available resources and patient/family needs identified by his/her staff to appropriate health care team members, in an effort to improve services for families.

  • Provides education and support for Resource Center Care Coordinators on designated patient care teams, assisting/guiding them in goal setting, process improvement and achievement of desired team outcomes as appropriate.

  • Meets biweekly with Manager of Social Work and Manager of Case Management to review identified issues and to work toward resolution and pro-active development of systems of improvement

Minimum Qualifications

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

Education: Masters of Public Health or similar degree in a related field from an accredited university.

Experience: Three (3) years of directly related work

Knowledge, Skills, and Abilities

These are the observable and measurable attributes and skills required to perform successfully the essential functions of the job and are generally demonstrated through qualifying experience, education, or licensure/certification.

  • Ability to guide staff in incorporating principals of Family Centered Care as a basis of clinical practice.

  • Ability to anticipate, assess, plan for, and manage physical, psychological, and social needs of the patient and family/caregiver and provide appropriate support, comfort, and reassurance.

  • Ability to apply a professional code of ethics and professional guidelines to clinical practice.

  • Ability to be a resource for staff.

  • Ability to integrate patient care with other members of the interdisciplinary health care team and provide direction or guidance to non-licensed personnel in assigned area of responsibility to ensure quality care.

  • Knowledge of computer systems and software used in functional area.

  • Knowledge of current theories, principles, practices, and standards in the health care system, and the responsibility and accountability for the outcome of practice.

  • Knowledge of laws, rules and regulations; standards and guidelines of certifying and accrediting bodies; hospital and department standards, protocols, policies and procedures in the area of assignment.

  • Knowledge of medical and professional ethics and patient privacy rights.

  • Knowledge of medical terminology; principles and practices of health promotion, risk reduction, illness and disease prevention and management.
