Overview

Our expansive network includes the flagship Carol G. Simon Cancer Center at Morristown Medical Center and Overlook Medical Center and comprehensive oncology programs at Chilton Medical Center, Hackettstown Medical Center and Newton Medical Center. Atlantic Health System Cancer Care has long been recognized as a leader in cancer treatments, earning numerous awards and accreditations for its oncology services. It covers 11 counties in northern and central NJ.

We are currently seeking a full-time outpatient infusion clinical pharmacist for Overlook Medical Center's Carol G. Simon and Advanced Care Oncology and Hematology (ACOHA) group. The main campus is Carol G. Simon where the outpatient oncology clinical pharmacist will spend majority of his/her time. All outpatient infusion centers deliver patient care beyond cancer, they are a place for to treat multitude of serious or chronic conditions, such as immune deficiency disorders, gastrointestinal track diseases, serious conditions like Crohn's disease, ulcerative colitis, multiple sclerosis and more.

The outpatient oncology clinical pharmacist collaborates with and serves as a resource for nurses, physicians and other clinicians who participate in the management of patients at Overlook Medical Center's outpatient infusions. He/she is responsible for coordinating and delivering pharmacy services in compliance with all applicable policies, regulatory agency requirements and state and federal laws. The position will be a hybrid role which will include staffing in the outpatient infusion center satellite (~60/40 split clinical/staffing.



Responsibilities

Essential Functions:

Job-Specific Responsibilities:

  • Function as a service-based clinical pharmacist as a member of the interprofessional team who proactively contributes to the decision-making process and the development of patient-centered care plans
  • Delivering satisfactory and valued services and finding mechanisms to reduce medical costs
  • Collaborates with prior authorization team to facilitate an appropriate drug selection is consistent with best practice
  • Navigates the realm of biosimilar medications and recognizes that different formulary preferences are necessary to safely transition from inpatient to outpatient, avoid interruptions in treatments or duplications
  • Educate patients and their families to ensure their understanding of the goal, administration schedule and toxicity management of chemotherapy treatment and documents the activity and interventions in the medical record
  • Identifies eligible patients for patient assistance programs to ensure continuation of care
  • Assess patient's accessibility of medications, risk evaluation and mitigation strategies (REMS) program, and specialty drugs
  • Collaborates with pharmacy manager in managing outpatient infusion metrics and identifies opportunities to improve quality and safety
  • Promotes AHS clinical research in oncology and collaborates with Pharmacy Research team to identify eligible patients
  • Participate in medication, treatment plan, and therapy plan builds in the Electronic Health Record.


Other Responsibilities:

  • Reviews and monitors patient plans (therapy and treatment) for appropriateness based on current clinical and institutional guidelines as well as patient-specific criteria
  • Identifies treatment issues and medication misuse
  • Assesses all medication orders to ensure they meet established standards and practice guidelines of AHS Cancer Center
  • Advises clinicians on drug selection based on current clinical guidelines to optimize drug therapy
  • Adheres to safety requirements in the handling and preparation of chemotherapy and other hazardous agents according to current guidelines (USP 797, ASHP, NIOSH)
  • Participates in education activities and answers drug information questions
  • Identifies, reports, and analyzes therapy errors and adverse drug reactions
  • Participates in the training, education and mentoring of other pharmacists and technicians and precepting of pharmacy students and pharmacy residents
  • Participates in quality, safety and other project work related to oncology service in the outpatient infusion setting
  • Conducts medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization with publication and/or presentation at local and/or national meetings
  • Assists in the development of policy and procedures
  • Participate in multi-disciplinary committees including Pharmacy & Therapeutics, Medication Management, IRB, Oncology related committees to support the appropriate use of medication therapy


Qualifications

Requirements/Education/Licensure:

  • Current NJ Board of Pharmacy License
  • Doctor of Pharmacy (PharmD)
  • Minimum of 3-5 years of experience in oncology with at least 50% time spent in optimizing drug therapy through the design, recommendation, implementation, monitoring and modification of individualized pharmacotherapeutic (treatment and support) plans in collaboration with the multidisciplinary healthcare team, and through the education and patients, caregivers, healthcare providers and trainees.

Additional Qualifications / Requirements

  • Board Certified Oncology Pharmacist (BCOP) preferred
  • PGY-1 Pharmacy Residency preferred
  • Experience in research activities preferred
