This position is aligned under the Pharmacy Service at the Robley Rex VA Medical Center. This position provides clinical pharmacy support to outpatient BHIP teams. The Clinical Pharmacy Specialist (CPS), Outpatient Mental Health (MH) uses expertise in psychiatric disorders and psychopharmacology to optimize patient care. The CPS functions as an Advanced Practice Provider and member of their assigned MH team working at the highest level of clinical pharmacy practice, autonomy and independent decision making under an individual scope of practice to directly care for patients. This position will be primarily focused on providing outpatient MH comprehensive medication management as part of a Behavioral Health Interdisciplinary Program (BHIP) team but will provide coverage for inpatient MH pharmacy services when needed. Evaluates the clinical status of patients using appropriate psychiatric assessment tools, including suicide risk assessments, patient signs and symptoms, review of systems, laboratory tests and diagnoses Performs comprehensive medication management including initiating, modifying, or discontinuing medication therapy Designs, implements, assesses, monitors and documents evidence-based, guideline-directed therapeutic plans utilizing the most effective, least toxic and most economical medication treatments Helps achieve positive patient-centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas Conducts physical assessments as needed to monitor the efficacy and safety of drug therapy Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy Orders consults (i.e. Nutrition, Social work, Prosthetics) as needed to maximize positive drug therapy outcomes Alerts BHIP team when disease progression or adverse drug events occur that require treatment interventions outside of their scope of practice Therapeutic drug monitoring and dose adjustment of inpatient psychiatric medications when covering the inpatient MH pharmacy service Work Schedule: Monday-Friday 8:00am- 4:30pm Compressed/Flexible: Not Available Telework: Not Available Virtual: This is not a virtual position.
