Under the leadership of the Director of Clinical Pharmacy Programs, the Geriatrics Clinical Pharmacy Specialist will work to advance the care of Geriatric inpatients within Baystate Health. This will include rounding with a multidisciplinary team on an Acute Care of the Elderly (ACE) inpatient unit and involvement and management of projects related to improving care and care coordination for this patient population. This Pharmacist will also perform a variety of duties including; age appropriate clinical pharmacy services, evaluation of medication orders for appropriateness, monitoring and patient follow up; drug information consultations, and support of the pharmacy department operations. The Clinical Pharmacy Specialist will also be responsible for conducting a layered learning model of preceptorship which includes APPE students, PGY1 residents +/- PGY2 residents. The Clinical Pharmacy Specialist will maintain competency, training, and ongoing education of pharmaceutical knowledge, integrated technology, and practice model changes. It is the expectation of the pharmacist to complete and document the appropriate tasks required to maintain patient specific record keeping in an electronic database. Such documentation may include Clinical Interventions, Safety Reporting Events, etc.
To qualify for an interview, the candidate must meet one of the following bulleted criteria:
One-year of post graduate training in a Pharmacy Residency program OR five years of progressive pharmacy experience
PLUS either PGY-2 or Fellowship training in an area related to geriatrics OR Certification in Geriatrics by the Board of Pharmaceutical Specialties (BPS) or Commission for Certification in Geriatric Pharmacy
All candidates must demonstrate successful project management and leadership.
Registered Pharmacist in the State of Massachusetts or eligible; Bachelor's, Master's or Doctorate in Pharmacy; Good written, verbal communication and presentation skills; Ability to critically evaluate patient therapy and drug literature; Ability to establish collaborative relations with medical staff and other health care professionals; Teaching experience preferred; Board Certification with the Board of Pharmaceutical Specialties strongly preferred.
BMC Full-time (1.0 FTE) position; hours consist mainly of weekday day shift with a rotating evening, night, weekend and holiday schedule.
