This position is responsible for HIM operational functions supporting the completeness, integrity and privacy of medical record documentation with a high degree of accuracy and attention to detail. Typical functions include document scanning, responding to release of information requests, and providing support for the dictation/transcription processes.
Scans medical record documentation into the electronic medical record:
Prepares patient documents for scanning process by verifying presence of patient data and quality of the document for scanning.
Scans and indexes documents to the appropriate order, encounter, or office visit in the electronic medical record with the correct document type and appropriate description according to protocol.
Responds to requests for protected health information (PHI) in accordance with federal and state rules and regulations, and Fairview policy:
Validates authorization to assure requesters rights to PHI
Processes requests in a timely manner using secure e-mail, CD, paper, or release of information module in EHR.
Escalates request to department management as needed.
Verifies and submits State of Minnesota required reporting (e.g. birth and death certificates).
Performs a wide variety of duties which may include but are not limited to:
Provides front desk support for department calls and visitors
Analyzes medical record for document deficiencies.
Retrieves and distribute patient medical records for scheduled and unscheduled patient care; re-file records and other documents.
Purges medical records according to guidelines and prepares documents for storage or destruction.
High school diploma or GED
Completion of computer training in Microsoft applications
One year in Health Information Management, medical office, or comparable work experience or education
Two years in Health Information Management, medical office, or comparable work experience or education.
