Clinical Pharmacy Specialist/Patient Aligned Care Team must be a licensed pharmacist, will serve as a provider with prescribing privileges as outlined in the individual's Scope of Practice for Clinical Pharmacy Specialist (CPS) in PACT. The primary responsibility of the Clinical Pharmacy Specialist is the provision of evidence-based pharmaceutical care services through both direct and indirect interaction with patients and providers. Promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. Functions as part of the PACT Team and is most effective when assigned to a panel of patients, to support continuity of care. a. Works with the assigned Patient Aligned Care Team (PACT) to optimize patient care b. Participates in multidisciplinary team meetings for ongoing development of pharmacy PACT program c. The monthly clinic utilization summary (CUSS) for all clinics requires 80% face to face for Satisfactory performance. d. The monthly clinic utilization summary (CUSS) for all clinics requires 80% telephone clinic for Satisfactory performance. e. The total monthly pharmacy encounters for all clinics (face to face and telephone) requires 140 encounters for Satisfactory performance. f. The total weekly patient appointments for all clinics (face to face and telephone) requires 30 hours for Satisfactory performance. g. Maintains data to evaluate program success on ongoing basis. h. Assists in screening patients for PACT pharmacy services i. Serves as a primary provider for patients in the clinical setting and assumes personal responsibility for successful clinical patient outcomes via prescription and monitoring of pharmacotherapy as per clinical privileges for PACT. j. In collaboration with the health care team, contributes to the design, implementation, modification and monitoring of cost-effective therapeutic plans for patients. Includes ordering and interpreting laboratory tests and clinical data to assist in monitoring patient care. Requires knowledge of and ability to adhere to formulary and therapeutic guidelines. k. Document all pharmaceutical care activities appropriately utilizing CPRS including all patient encounters, significant findings, assessment and plan. l. Delivers effective education to patients, the public and healthcare professionals. Provide concise, applicable, and timely responses to requests for drug information from healthcare providers and patients. m. Appraise existing drug therapy of patients to determine the appropriateness of drug, dose, route/frequency and method of administration. When appropriate, will assess, document and intervene for concerns such as compliance, drug interactions, therapeutic duplications, therapeutic outcomes, formulary adherence, and cost. n. Identifies, interprets, and resolves prescribing and dispensing issues in a timely manner. o. Participates in the medication reconciliation process by interviewing patients and documenting medication use including facilitating communication with appropriate healthcare providers when conflicts are identified and by documenting non-VA medication as per medical center policy. p. Works within the assigned scope of practice along with completing all progress notes in a timely manner per hospital policy. q. Coordinates patient care with primary care and specialists when appropriate. Work Schedule: Full-time, permanent, Monday-Friday (8 hour shifts between 7 am - 5:30 pm)Financial Disclosure Report: Not required
