The Charles George VA Medical Center, Asheville, NC has an immediate opening for three (3) Pharmacy Technicians (Clinical Medication Reconciliation) within the Pharmacy Service. This technician, as a member of the pharmacy clinical staff, functions under the matrix supervision of the assigned Inpatient Program Manager and the Associate Chief for Clinical Pharmacy. The primary functions of the Clinical Pharmacy Technician include medication history taking, prescription medication review, laboratory value monitoring, medical chart / pharmacy profile poly-pharmacy reviews, medication use evaluation monitoring, pharmacoeconomic reviews, telephone logistical support. On-site certification of clinical competency shall be completed prior to performing assigned clinical functions. Assigned activities include: Routinely reviews the computer medication profile for prescription duplication, therapeutic duplication, prescription noncompliance, and allergies/ad verse drug reactions. Reports identified problem patients to the assigned clinical pharmacy specialist for evaluation. Routinely reviews laboratory monitoring reports for selected disease states and identifies patients with values outside of the normal limits. Reports identified patients to the assigned clinical pharmacy specialist for evaluation. Routinely performs patient medical chart and medication profile reviews to identify poly-pharmacy problems. Reports identified problems to the assigned clinical pharmacy specialist for evaluation. Completes clinical pharmacy medication utilization evaluations and protocol-driven clinical pharmacist peer review. Provides supplemental reinforcement education for patients concerning operational issues or the need for compliance with therapy and retrieves data points for clinical pharmacy specialist review. Documents clinical and operational activities as assigned. Participates in Performance Improvement processes and programs as assigned. Work Schedule: Monday - Friday; Hours 8:00 a.m. - 4:30 p.m. Financial Disclosure Report: Not required
