The Clinical Pharmacy Specialist (CPS) for primary care Patient Aligned Care Teams (PACT) is a multifunctional position with primary emphasis on pharmacotherapy, including prospective drug use evaluation, comprehensive medication therapy management, and treatment planning to improve drug selection, dose, decrease reactions and interactions, and promote rational use of therapeutics.
