GENERAL SUMMARY:

The Rehab Liaison is a multi-faceted position.  It includes evaluating and coordinating outside and assist with in house referrals to the rehabilitation unit, as well as implementing and following up on marketing activities to physicians, case managers, social workers, and other referral sources.  The liaison is responsible for increasing the rehabilitation unit’s current volume by building and maintaining relationships with existing and potential referral sources.  The Liaison may be asked to inform referral sources of other services and programs provided by the hospital.  Performs other duties as assigned.

PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:

 

  • Coordinates appointments/ luncheons to physicians and office staff, hospital case managers/social workers/discharge planners.
  • Assists in implementing community events involving referral sources.
  • Works with insurers and managed care companies to educate about Rehab services and capabilities.
  • Provides information to patient and family and significant other regarding rehab process.
  • Promotes a positive relationship with referral sources (i.e. physicians, case manager and the community).
  • Networks with families and patients to explain rehab services and reimbursement.
  • Tours families through facilities.
  • Coordinates outside admission referrals and follow-up with referral sources to ensure proper communication throughout the admission process.
  • Increases referrals from existing sources and new referral sources by educating about programs/services.
  • Calls on referral sources each week.
  • Sponsors/hosts on-site functions with referral sources to attract business.
  • Identifies and reports competitive services to the team.
  • Prepares statistical reports and tracks information for management regarding referring physicians, hospitals, and diagnosis of patient, reasons for denials, total referrals.
  • Reviews medical records, recommends appropriate rehab setting based on medical necessity, patient needs, physical capability, cognition, age level and rehab potential.
  • Composes marketing brochures and collaterals, including case studies to share with referral sources.
  • Communicates recommendations to Rehab physicians, social services, and nursing staff and provides completed assessment upon admission to unit.
     

 

Qualifications

KNOWLEDGE AND SKILLS REQUIRED:

 

  • Knowledge of CMS Regulatory Requirements
  • Medical Terminology
  • Interpersonal Skills
  • Communication Skills
  • Marketing Ability

 

KNOWLEDGE AND SKILLS PREFERRED:

  • Any combination of education, training or experience that provides the knowledge, skills and abilities required to successfully accomplish the assigned duties and responsibilities of the position.

 

EDUCATION AND EXPERIENCE REQUIRED:

  • Licensure in a healthcare related field- RN 
  • AA In Nursing

EDUCATION AND EXPERIENCE PREFERRED:

  • Bachelor in Nursing
  • Three years healthcare related experience
  • One-year sales/marketing experience or experience in acute rehab marketing
