Job Summary

Provides support to the Anticoagulation/Medication Therapy Management Clinic.

Job Duties

  • ​Provides clerical support for all aspects of the patient's visit to the clinic including but not limited to: scheduling functions using the Cadence scheduling program, contacting patients regarding appointments, and retrieving patient information and demographics.
  • Triages phone calls and responds to e-mail, EPIC, and voicemail messages.
  • Creates telephone encounters within EPIC to document communication from a patient or care team member to pharmacists and other health care providers, including but not limited to: requests for new lab order, medication refills, and patient questions.
  • Retrieves lab results and outside facility records to be filed in patient chart.
  • Schedules home phlebotomy, home health visits, and outside laboratory draws.
  • Assists in department operational activities such as tracking missed referrals, ordering and replenishing stock, and distribution of clinic mail.
  • Communicates with Client Services, outside laboratories, pharmacies, and physician offices regarding issues related to patient care such as lab draws/results, prescription medications, and all other requests per the supervising pharmacist.
