Overview

Fairview Home Infusion is a nonprofit provider with over 25 years of experience, offering personalized home infusion care for adults and children of all ages. Our mission: Helping people of all ages manage their illness while continuing to live their lives. We care for patients across the five state areas of Minnesota, Wisconsin, Iowa, North Dakota and South Dakota.

We are looking for a full time RN Clinical Coordinator-Home Infusion to work at our facility in Minneapolis. This is a benefit eligible, day position that includes rotatingweekends.

Responsibilities/Job Description

Assure fast, easy access to Fairview Home Infusion (FHI) services as an expert infusion therapy resource for referral sources, nursing agencies, patients and FHI staff.

  • Identify and coordinate the admission of appropriate patients to FHI services to assure a safe environment for home infusion therapy. Provide patient assessment based on established criteria.
  • Demonstrate knowledge about home care and infusion therapy rates and policies; negotiate pricing under guidance of the Supervisor.
  • Collect, aggregate and analysis of data on IV device related infections for the patients in the Fairview Home Infusion program. Generate action plans with the Nurse Manager or nursing agencies if trends or results are greater than established thresholds.

Identify and contract with nursing agencies outside the metropolitan area that will provide quality nursing care to patients who receive infusion therapy services from Fairview Home Infusion.

  • Ensure that each contracted nursing agency complies with accepted standards of care and state licensure requirements.
  • Oversee and coordinate appropriate clinical follow-up for patients.
  • Approves charges submitted by nursing agencies as needed to ensure accurate, appropriate and timely billing of services provided. Responsible for authorization of all visits made by nursing agencies and Fairview affiliate home care agencies.
  • Assures that all appropriate documentation is present in the patient's medial record.

Perform insurance benefit checks and responds quickly to referral sources (within 2-4 hours, 24-28 hours for late or weekend referrals).

  • Actively participate (teamwork) in decisions regarding payer information and the cost to perform service with the Nursing and Pharmacy Infusion Therapy staff.
  • Perform verification of insurance benefits and document appropriate sequencing of payers for accurate billing. Notify patient/caregiver of anticipated financial responsibility.
  • Secure initial authorization or certification on third party referrals. Obtain the initial authorization for all new referrals before If authorization cannot be obtained before visit is due, call the referral source/physician to notify and call patient to inform of possible financial liability.
  • Screen referrals for reimbursement source and actively question or problem solves discrepancies between services ordered and reimbursement information.
Qualifications

Required:

Current RN licensure in state of Minnesota

Minimum of two years in home infusion services.

Preferred:

BSN
